Requirements to get a Philhealth ID
- Two accomplished copies of Philhealth application (download here)
- Two (2) latest 1×1 ID pictures
- Two valid IDs or a photocopy of your NSO Birth Certificate
Present the requirements presented below at any PhilHealth Local Insurance Office (LHIO). Members can claim the ID at any PhilHealth LHIO for Php 90.
If you wish to apply as dependent for your ID card, just pay Php 358 at any Bayad Center outlets or at any SM malls for Php 360.
